information regarding their Covid Booster clinics.

·         Please note that the Clinic is not limited to just State
employees and their families, so feel free to share this information with
any interested party.

·         Walk-in’s are welcome but you can sign up ahead of time at the

*BOOSTERS AND FULL VACCINES AVAILABLE:*  All three formulations of vaccine
(Pfizer, Johnson & Johnson, and Moderna) will be available every day,
including boost doses for those that are approved to receive them.

·         *To be eligible for a Pfizer or Moderna boost, you must be at
least 6 months out from your second dose.*  To be eligible for a Johnson
and Johnson boost dose, you must be two months from your last J and J
vaccine.

·         Full Vaccinations are available for all individuals ages 12 and
up, but minors must have an adult present in order to be vaccinated.

o   The Clinic will be able to support vaccinations for all boost doses as
well as children ages 5-11 once those additional groups have been approved
for vaccinations.

·         Please note, when using the Consent form link above, *those
registering for their boost dose need to select first dose*, if they don’t
see boost as an option.  IT is adding the boost dose option, but it’s not
there yet.

·         Take your COVID vaccine card with you for faster processing on
site.

·         When you get to the museum, be sure to look for the COVID
Clinic.  There are several clinics listed in the location, but some of
them, such as the FLU clinics, are only open to State employees.
