Subject: 65+ Get Help Signing Up for the SNCOHD COVID-19 Vaccine Interest
Survey - Mobile Pop-Up Clinic, Saturday March 6th

A large majority of our 65+ neighbors covered under the SNCOHD COVID-19
Phase 2 vaccination plan are unable to complete the required interest
survey which is mandated to receive a vaccination appointment.

In an effort to ensure equitability for all of our 65+ neighbors we are
scheduling vaccination survey interest form pop-up clinic events throughout
the community. We estimate the survey completion time of 2-5 minutes per
participant. In addition, each participant will receive a resource bag
containing community resource information and a free 20 oz. bottle of hand
sanitizer.



Social distancing mandates will be adhered to by all during the events in
an effort to ensure everyone’s safety. Participants will be required to
wear a facial mask and remain in the car.
